What version of cacti are you running? The INSERT would suggest that it's trying to insert a new record but using all the same details as an existing record so it fails it.

Unfortunately, there have been quite a lot of changes between 0.8 and 1.1.38 which is the current version. I don't know if others have had the same issue but I don't know enough about 0.8 to make safe assumptions at the database level.
